The Improvement Foundation has been working to transfer knowledge on running collaboratives to the Health Quality Council in Saskatchewan province in Canada.
This has resulted in the Saskatchewan Chronic Disease Management Collaborative with frontline healthcare professionals, managers and support staff to improve the care for nearly 8,000 patients with diabetes and coronary heart disease and improve patient access.
The collaborative is a partnership that includes all 13 health regions, 73 family physician practices (including office staff), First Nations’ organisations, and community health care providers.
The first part of the Collaborative was launched in November 2005 and ended in February 2008, involving 35 practices.
The second group started in November 2006 and will finish in March 2009. This involves 38 practices.
