Reviewing Patient Medication – Northampton
A pharmacist member of the Northampton site carried out a review of medication for people in a residential home, following their agreement. All residents – who were aged from 74 to 91 – were taking between four and 10 different medications. Each resident was taking at least one drug with the potential to cause unsteadiness. The pharmacist contacted each patient’s GP to discuss making changes.
