Reviewing Patient Medication – Easington
Taking four or more prescribed medications can increase the risk of an older person falling. A steering group member from the Easington site compiled a list of all those over 65 who belonged to a particular GP practice and then wrote to the practice manager at the local surgery to ask them to review their medications.
The practice reviewed five patients a week and contacted the patient to discuss changing their medication where appropriate.
