Patient Involvement – Enfield PCT

To facilitate with their long-term conditions management and the production of their self-care strategy, Enfield PCT held two process mapping events involving patients.

Process Mapping Exercise 1:

  • Patients with diagnosis of a long-term condition, such as diabetes, heart conditions and Parkinson's disease, were invited to a process mapping meeting.
  • Patient mapped their own journey from diagnosis to present.
  • ‘Bottlenecks' in the process were identified as were the steps that have no added value for the patient.
  • The patients were then asked to think about how the process could be redesigned.

Discussion took place about how the diagnosis is explained, what information is available and how it is accessed, understanding of the information provided, and what could be done to improve information and education to help with self-care.

Process Mapping Exercise 2:

  • Looked at self-care, education of patients and education of clinicians in relation to self-care.
  • Looked at the information available.

Both these events were enjoyed by patients and staff and have helped to contribute to future local health care.
