Long-term Conditions Team - Durham Dales
The team consists of four community matrons, two social workers and one patient facilitator. The team is based in the same office as the intermediate care team, and some specialist nurses. Each community matron is responsible for one of the four localities with a total of 13 GP practices. Locality populations range from the highest levels of urban deprivation to extremely rural, widespread population. During a two-year pilot period, the team has seen a 15% drop in unplanned hospital admissions and 20% drop in bed days for the entire over-65 practice populations.
